Online Initial Goal Setting Report

This report is designed to be completed by the learner and the tutor at the start of their match.

 

The purpose of this report is:

 

  1. To provide a tool to assist our learner-tutor teams, as they start their match, to be clear and focused about what they will work on together.
  2. To inform LV program coordinators on the activities, needs and goals you will be focusing on in your time together.

Referrals
Although the focus of the LV tutor-learner match is literacy, we recognize that other issues may arise from time to time. If non-literacy issues arise, LV supports referrals to appropriate agencies. Your program’s coordinator may be able to provide information on community resources to assist with non-literacy issues.
Goal Setting
When people write their goals down and develop a plan for achieving these goals, they are more likely to be successful. Literacy Victoria supports and encourages our tutors and learners to set goals. The most effective goals are S.M.A.R.T.I.E.S.– Specific, Measurable, Attainable, Realistic, Time limited, Imaginative, Enthusiastic and Shared. Goals can be related to independence, learning (school) or work.
I understand Literacy Victoria is committed to protecting the privacy of personal information in our possession or under our control in accordance with the Personal Information Protection Act (PIPA). PIPA regulates the way we collect, use, keep, secure and disclose personal information. The information collected on this form will be stored in locked office and/or in our secure electronic database. Literacy Victoria values the trust of our donors, volunteers, clients, participants and staff.
