This report is designed to be completed by the learner and tutor together.


The purpose of this report is:

 

  1. To keep LV program coordinators informed about the activities, needs and goals of the learner-tutor teams.
  2. To provide a tool to assist our learner-tutor teams to be clear and focused in their work together.

Referrals
Although the focus of the LV tutor-learner match is literacy, we recognize that other issues may arise from time to time. LV supports referrals to appropriate agencies to address the non-literacy issues. Your program coordinator may be able to provide information on community resources.
Goal Setting
When people write their goals down and develop a plan for achieving these goals, they are more likely to be successful. Literacy Victoria supports and encourages our tutors and learners to set goals. The most effective goals are S.M.A.R.T.I.E.S.– Specific, Measurable, Attainable, Realistic, Time limited, Imaginative, Engaging and Shared. Goals can be related to independence, learning (school) or work.
I understand Literacy Victoria is committed to protecting the privacy of personal information in our possession or under our control in accordance with the Personal Information Protection Act (PIPA). PIPA regulates the way we collect, use, keep, secure and disclose personal information. The information collected on this form will be stored in locked office and/or in our secure electronic database. Literacy Victoria values the trust of our donors, volunteers, clients, participants and staff. We recognize that maintaining this trust requires accountability and transparency in handling personal information.
